Go to: Searches
Click: Search as shown…

Split screen and leave search results in the left.
For each search, under the “Documents” section at the bottom, there should be an Authorization for Release of Limited Information to Third Parties document entitled WC or PreSearch or PS____ sometimes with the last name included.
Open this form in your right screen.
Check to see:
If the claimant name matches the name of the subject in the search.
If the SSN matches the SSN in the Search.
If the date signed matches the date it was “sworn” / notarized.
If the notary's commission is up-to-date.
*If any of these do not match, leave a client note asking for clarification. Wait for clarification before completing next steps.
If all are okay…
Download the Authorization form and name it WC_Last Name
Repeat this process for all open searches in the queue.
Now, in your right screen, open Colorado WC Form
Choose today's date.

For all searches that are ready to send, highlight and drag the full name as listed ans SSN to the appropriate boxes in the Colorado WC form.

We typically have 1-4 per request, but the form will allow up to 10. If you have more than 10, you can just complete an additional form and send everything together.
Once the form is complete, click submit.

Then click “Download your PDF”

Then go to your downloads and chance the name from
“Ready-Fill WC Form” to WC_Today's Date
Open Support Email.

Back in search results,
Click: Create Note

Add Comment in Admin Note:
Emailed required docs to cdle from support.
Click “Save Notes”

Once we've received a response from CDLE…
Open the email, “trust sender”
Screen Snip the SSN, Name, and any additional report information if there was a claim or multiple claims for one subject. Save as CDLE Results.
Most common: “NO CLAIMS FOR THIS SSN”
If no claims, click “Upload Documents for this search.”
Save the screen snip to the search level.
Click “No Record”

Search will disappear from the queue. It's finished.
If there are one or more records:
Snip, save and upload results as usual.
Check the (DOI) date of incident.
If it is as recent as 10 years ago or less, report it. If it is older than 10 years ago, make a note “not reporting; older than 10 years” and don't report it.
For those you need to report:
Click “Record”

The File Date is the same as the Injury Date which is listed from the emailed report as the DOI (Date of Incident) These all mean the same thing. Enter File Date, SSN, Injury Date, Injured Employee's Name, Company Name, and Desc Of Injury carefully and correctly. For Employer's Insurance No: Enter “N/A” Do this also for Employee's Current Address. Leave all other fields blank as they are not required. Then click “Enter Record”

An automated window will pop up to “enter this case?” Choose OK.

Double check all information.
Click “Finalize And Submit RECORD”

Check again.
Click “Submit RECORD”


Click OK
You will see the record in the queue with notes “Sent to Quality Assurance Department” until it is cleared by QA.
